Baptism Registration Form

Once your registration form has been submitted and reviewed, you will be contacted with complete instructions and details for the day of your baptism. Registration should be complete at least two weeks prior to the baptism date.

Fill out form below and submit online or print the form, complete it, and submit it to the church office.

Please note: Children in grades K-5th grade desiring baptism should fill out the alternate KidCity baptism registration form when requesting baptism.
